Here is what I got in debug mode:

Dec 16 11:33:14 rudi postfix/smtpd[7514]: ctable_locate: install entry key 109.212.245.::ffff:208.bl.spamcop.net

It seems ::ffff: prefix is not stripped before generating domain name to query.
