IMHO coś masz nie tak z PAM-ami (trudno mi bez wiedzy co masz w 
konfiguracji np. /etc/pam.d/sshd powiedzieć co), a do tego masz włączoną 
pewnie w konfiguracji sshd autentykacje na PAM-ach. Ponieważ z jakiś 
przyczyn PAMy Cię nie wpuszczają ssh przechodzi (po 3 próbach) do 
"standardowych" metod i tu jest wszystko ok. Zobacz czy nie masz w 
/etc/ssh/sshd_config czegoś w stylu "PAMAuthenticationViaKbdInt yes", a 
jak masz to albo wyłącz (wykomentuj), albo napraw pamy albo przyślij 
configi może coś się da wymyśleć.
